AFO took his Quirk, then gave him a different one where the doctor modified it, so it only destroys. It's explained in chapter 419 if you're curious :P

BIG MHA SPOILERS (chapters 423+) . . . . . . . . . . . . . . . Learned why Shigaraki was always itchy. I am not okay. It was because of the quirk AFO gave him. Just like Midoriya kept breaking his bones and Aoyama's stomach couldn't handle the laser. Their bodies were not made for it. Gonna sob now.
