This reminds me of when I was 14 and had testicular torsion. I ate a lot of bread prior to a home visit by the GP before being informed I would need emergency surgery. I was on the operating table for hours before they even attempted surgery.

The idea (in the UK) is supposed to be that service charges are tips ie: above wages. It's well known to put cash on the table, even with service charges - restaurant chains get vilified regularly for screwing folk out of their tips. They still do it, obviously.
