What happened to her to get struck with an axe? D:

I honestly have no clue yet! That's been her mystery throughout the whole campaign. I described that it happened, but left it up to my DM to fill in the details. I have a bit of an idea though, because pre-axing she was quite arrogant and on a power trip, plus an executioner for hire.
