It's a reference to Joe vs The Volcano. It's the made up, terminal disease Tom Hanks' character Joe Banks has...or believes he has, anyway. "Blotchy and puffy" is how he feels all the time because he works a shitty job in a windowless room lit by fluorescent lights that suck his soul out of him.
