As near as I can tell from talking to some, they mostly see things... in a way that I really can't for more than a second or so. They think "inner monologue" is a metaphor the way I used to think "mind's eye" was. These were very smart, successful scientists and engineers, so both ways work.
I learned this when talking to a senior engineer at my research lab 30 years ago, when he said something about how "of course 'inner monologue' is just a metaphor". We were both floored to learn what goes on in the others' head.

We need to standardize emojis for this. Maybe something like “🙅♂️❓🙅♂️”.
I do love the projection of the person (I presume one of many) who said, paraphrasing, that she was "just a boy who wanted to sneak into the girls' locker room" juxtaposed with the reality of her teammate saying "nobody ever used the locker room".
