I usually ask permission first unless they're being disruptive or an asshole at which point I feel like it's more fair game. With friends I feel OK just taking a shot if I'm capturing a moment. The person who was taking creepy videos of women in Eugene pissed me off and I blocked his shots.

Ask for consent. Always. Treat others how they want to be treated; don’t assume you know. If someone accidentally gets in frame, that seems different, to me. If the person/s are the focus of your lens, see above.

Unsolicited photos outside your friend group are generally not cool. People are there to have their own good time, not to amuse or thrill you. If someone's outfit or dance MUST be documented... ask.
