Another way to do this is to have multiple names for a place. Why shouldn’t humans, halflings, elves, dwarves, and orcs have their own names for places? Again, easy to say, hard to do.

I tend to get Simarillion brain-freeze if things have multiple names. "This town is known as Khiz-Koran amongst the dwarves, Santoregnia by the people of the mountain and Little Buttwhistle amongst the halflings.."
