This is insane. I work for city government, and one thing we talk about a lot is how federal law prohibits discrimination based on language. That is, we have a duty to provide a translator or a translated copy of documents. We cannot deny service bc of language differences.

I'll just add, the easiest workaround to this is to provide instructions in several languages. A separate sheet of paper that can go with the form. That way, there's no change to the official form, and you still provide services and accommodations to those who use languages beside English.
