The most common system I run (Pathfinder 2e) has rules for crits, so there's some adjudication, but the rules cover it for the most part. One of my favorite crit rules is the crit tables from Rolemaster. Nothing does crits quite like those.
Depends on the table for D&D 5e. Most of my games I just run it RAW (roll twice as many dice). Our Vengalia campaign uses the max+dice variant rule (1d10 weapon would be 10+d10), which is particularly spicy. I run a westmarches game that inflicts lingering injuries on crits.
I had a friend who ran us through a Savage Worlds game once, pre-pandemic. Sadly I don't remember much about it, but I do remember it being a bit crunchy. - T
