I think one thing that could do it is if there's another underground space deeper down nearby that water can drain into. Could be draining into a hidden cave or underground river. If not, an exceptionally badly designed well.

I really wish Unity had made it more clear what the "Pixel Error" setting is, in the context of terrains... It's described as "The accuracy of the mapping between Terrain maps and generated Terrain." What it does, is it adjusts how quickly it switches to lower LODs for terrain.
