One of the first campaigns I wrote, the party started out in a cult town. On their way out they tried to kill as many cultists as they could, but one guy both couldn't die and couldn't effectively stop them. Thus was born the recurring character of Ineptitude Joe, the dumbest cultist in the world.

Sadly there aren't many monotheistic religions around to draw inspiration from. Unless you're basing things off of Zoroastrianism, it's either Abrahamic faiths or bust...

If anything, having disabled characters makes your worldbuilding more real and believable. Working out how disabled people would interact with your world is just as interesting as working out how able bodied people interact.

In theory yes, but the idea of someone's magical muscles breaking down and causing them such severe pain is a little disquieting since non-magical doctors and healers wouldn't know anything is wrong.
