Sounds really positive as I’ve been trying to encourage the PCN to allow their pharmacists to reconcile discharge meds What’s the “techy bit” of referrals as I’m keen to know more about what GP assistants can do?

I always read the discharge summary first - so I can disagree with meds changes if they seem inappropriate - but then I forward the letter to our pharmacy team to reconcile meds, sort out dosette boxes etc. It a big chunk of work I'm happy to hand over.
