The amount of times this has happenes...
The 2nd is legit why I am kinda done with getting commissions. I have a very short list of friends I'd trust to get art from again, but there's been so many others where its 3x the deadline estimate with 0 updates unless I hound them.
When KingCheetah died my comment was that his outstanding commissioners were going to have to wait only slightly longer than usual
And that is why I avoid accepting early payment unless I'm in real need of it. Hate leaving people with the feeling of being scammed or desperate, plus refunding would be a headache with how paypal + brazil is.

I once waited 5 months for a YCH, with the excuse that the artist was extremely backed up. I say excuse because as soon as I threatened to charge back they suddenly finished it within a day. There’s others I’ve totally forgotten about from years ago that I remembered cleaning my FA notes
And that tricky timeline of when it’s acceptable to nudge them.
For real tho. I've had comms I've waited over a year for.

Sometimes I entertain the thought of doing a charge back when no WIP has been sent to me by then without any discernable reason given and be like, look, if you're not planning on working on it, I'm not planning on paying for it
