The key thing is picking your duchy buildings. Most give a percentage-based boost (e.g. X per cent more levies for this duchy). Which is why you get the most benefit from focusing duchies, rather than balancing them.
Might is right. Obviously if the county/duchy has good economy options (e.g. farmland and trade ports) you change strategy. Then go money. But in most cases creating a strong powerbase is key early on.
Not always, but it's not uncommon. An entirely smooth succession is pretty rare. The trick is to try and make sure you have a bit of gold for emergency mercs. And also to optimise the primary duchy/county for your main title for levies.
