I'm not sure about this. Titles don't create hierarchies, behaviours & leadership styles do. Titles are indicators of responsibility (people ultimately need to know where the buck stops). Inclusive, less hierarchical orgs are created through behaviour & conscious intention.

Well we have the latter too. And I think titles = hierarchy has been a thing so long as we've had civilisation. And if there is clarity and alignment on mission and roles, everyone knows where the buck stops.
