I think this is badly written. The issue is not that there is no CPT code. The issue is there is no CPT code because insurers will not cover the surgery. Therefore no CPT code for the surgery exists. If you have no intention of paying for something you're not going to create a billing code for it.

I used to bill procedures & fight denials from remit advice. I guarantee there is a icd & proc code. Proc code likely mutually exclusive with patients DOB, like, the system auto-denies b'c patient is under 55 or something. Insurance Co won't tell you that.
