I hear you. I think a lot of the time. Not much thought goes into it. They only kinda work if they're very insular/isolationist. Otherwise it's kinda odd the world around them goes through massive shifts and they kinda just chill until things get really bad.

Most of the time the fantasy setting feels like they're in an age of decay. I can't really imagine a setting where you adventure while all those ancient power things and dungeons are being made/maintained.

Yeah a more nationalised healthcare system pretty much removes the need for a lot of independent hospitals Where I live now it's also universal but structured different. It's a public insurance so based on income rather than health conditions.

Depends on what you mean by universal healthcare. If you mean something like the NHS in the UK kinda but private healthcare is still used (privately accessed or via universal healthcare because of capacity issues)
