There was that mildly famous one on one of the pen fora, where someone was bitching about several TWSBIs breaking on him in a row, and everyone getting agitated about their quality control and it turned out he had knocked every single one off his desk at some point...

from what i've understood of the problem is that ppl screw on their caps too tight
