Yes that’s my experience as well. Their guidance is pretty clear that ‘exceptional’ really does mean just that. As an aside, depending on facts, that’s what the appeals process is for. But it’s daunting / expensive with lawyers.

Expensive enough to get through the process without lawyers. I regularly have to have conversations with people to explain that if you want a decision expedited because a relative is sick abroad, you will need medical proof that they are close to death. And then, maybe the HO might help.
