I just had a (expensive) drug I've been taking for THREE YEARS sent back for prior authorization. Yes, I still have the same chronic condition my doctor has submitted previous prior-approvals for, it hasn't miraculously vanished.
I think profit motive only works when you have options to move to another service provider.

A profit motive _never_ improves quality. The only thing that does is competition, and the market mechanism for doing that in health care is reckless and dangerous. "Well, after Doctor X lost 15% of his patients, people starting seeing Doctor Y instead, who only lost 10% of his patients."
It took me two years to pay off an ER visit because they coded my migraine wrong so insurance wouldn’t cover it. Whenever I have a should-go-to-the-ER migraine, I do not go anymore!
